5,300 years ago, an injured and starving man from Val Venosta, Italy, fled across an Alpine glacier to escape pursuers. But his enemies caught up with him and with a practiced arrow-shot penetrated his shoulder blade and subclavian artery. Before long, he was dead from blood loss. "Oetzi the Iceman" was found mummified in 1991. On his person were pouches containing mushroom remedies, the oldest known proof in existence of medicinal mushrooms usage.

There is good reason to believe he carried both of these mushrooms along as natural remedies. Whipworm parasite eggs were found in Oetzi's intestines. Birch polypore mushroom Piptoporus betulinus is a traditional de-worming remedy. The other mushroom in his possession, Tinder fungus (Fomes fomentarius), may have been used to cauterize the wound on his right hand.

Both of these are polypores, so named because they have pores instead of gills underneath. No species of polypore is known to be poisonous. They usually grow on trees, dead or alive.

Grzywnowics in a 2001 article titled Medicinal Mushrooms in Polish Folk Medicine said Agarikon tea was traditionally considered an elixir for long life and listed historical uses to include lung conditions (coughing, asthma) and rheumatoid arthritis, but also infected wounds and open bleeding.

Up to this point, we've only covered the use of medicinal mushrooms in the West. However, their use has been far more widespread in Asia. There are at least three Asian species that would be criminal to leave out of any article on the history of medicinal mushrooms.

First in this list has to be Reishi, a mushroom so revered throughout Asian history that it's found in numerous ancient wood-carvings and temple engravings. It also owns the distinction as the oldest medicinal mushroom in Chinese literature, first encountered in Shen Nong's Herbal Classic, 2,000 B.C. Often referred to as a "panacea, its acclaimed properties are too numerous to list.

Next is Cordyceps, a minute fungus from the Tibetan plateau, parasitising on silk caterpillars. Cordyceps sinensis was first mentioned as a medicinal mushroom in The Classic Herbal of the Divine Plowman from 200 A.D. It is popular with athletes to improve physical stamina. Historically, it's been used as an aphrodiciac.

Finally there is the Shiitake mushroom, which today is a common household name even in the West. In Asia, however, it is known to have been cultivated as a gourmet mushroom for at least a thousand years. What may surprise is that Shiitake is also one of the most researched medicinal mushrooms in the world. Its potential uses range from energizing tonic to immune-booster and antibiotic with anti-tumor properties.

Medical research on mushrooms appears to have begun in the late 1960's in Japan. It gained attention in the West through the research by Dr. Ikekawa, who found that families growing mushrooms had lower cancer-rates than other people in their communities. Since those early days, medical research into mushrooms as grown exponentially and is still increasing. Medicinal mushrooms are continuing to make history.

Osteochondral Allograft, Microfracture, OATS, and ACI Surgery of the Knee: Cartilage Restoration

Biologic treatments are best for knee disease or injury. Biologic treatment restores the knee to a near-normal state. Meniscal repair, reconstruction of the ACL (anterior cruciate ligament), and anatomic knee fracture repair are 3 of the types of biological surgical repairs that work well for knee injuries.

A symptomatic full-thickness chondral lesion of the knee presents a problematic management issue for patients and orthopedists.

Injury that leads to articular cartilage cell death is well treated by restoration of the surface cells of the joint. The shiny white surface that composes all the joints in the body is the articular cartilage or surface cell layer of the joint.

There are 4 ways to treat damage to this tissue. They are: Autologous Chondrocyte Implantation (ACI), Osteochondral Allograft, Osteoarticular transfer system (OATS), and Microfracture.

In a young patient, a small lesion can be treated with Microfracture surgery. In this method, a pick-like tool is used to enter the marrow of the knee under the chondral defect. Multiple entries stimulate the bone marrow, which, in turn creates repair tissue. This tissue fills the chondral defect with fibrous cartilage tissue.

In this situation, the patient will use crutches for four to eight weeks. The patient must agree not to participate in sports for 6 to 12 months. Also, the patient must understand and accept that it may be eighteen months before complete freedom of pain can be expected.

There is a procedure that can restore the knee surface to almost normal condition. This is called autologous chondrocyte implantation (ACI). This procedure is used in the case of large knee defects.

Articular cartilage cells can be harvested from the healthy part of the injured knee for utilization in ACI. There are very specific criteria that must be met for this surgery to be used.

These conditions are: 1. A full-thickness, weight-bearing, symptomatic chondral injury of the femoral articular surface. 2. A physiologically young patient. 3. The patient must understand and agree to participating in an 18 month rehabilitation process.

There is no guarantee of successful surgery to the tibia and patella. In fact, insurance companies will often refuse to pay for this type of surgery. It is important to note that ACI is not a workable procedure for the treatment of osteoarthritis. In this condition, two reciprocal joint surfaces experience damage. X-rays will reveal narrowing of the joint space. Additionally, bone spurs will be in evidence. 3T MRI (magnetic resonance imaging) may be used to assess for ACI. By using this procedure, the proper treatment can be determined.
